1. Complete the form and hit the "Submit Form" button at the top right of the screen.

2. You will be prompted to fill out your email address and your name then hit "Send"


3. You will now be asked if you wish to submit your form via "Desktop Email Application" (Outlook, Eudora or
Mail for Mac) or "Internet Email" (Yahoo, Hotmail, Gmail etc. - you visit a website to access your email account)

If you select
"Desktop Email Application" - your email will automatically be opened, a new message created and
the completed form will automatically be attached. All you have to do is click to send your email. You're done.

If you select
"Internet Email" - a Save As screen will open. Under file name or save as put your name then
indicate that you want to save it to your Desktop. Then click "Save" button

Compose a new email to: monika@hqacupuncture.com
Subject line: COMPLETE FORM
Attach the file you just saved on your desktop and send it to us
